Case Management
Successful reentry starts with a plan. Our Parole Planning and Case Management services guide participants step-by-step through the parole process and connect them with the resources they need to succeed.
Case managers work with participants to:

Develop individualized reentry plans

Coordinate access to housing, employment, education, and transportation

Provide support navigating legal obligations and appointments

Connect participants with counseling, life skills, and support networks
These services help participants avoid setbacks, make informed decisions, and build a sustainable path to independence.


Welcome Home
When an individual joins the On The Way Home (OTWH) program, we make sure their most basic needs are met. Each participant receives a welcome Home bag containing hygiene items, clothing, shoes, towels, a prepaid cellphone and phone card if needed, as well as food items or food card.
At OTWH, we believe that a successful reentry begins with the essentials — food, safety, and dignity. You can be part of this effort by donating food items or volunteering to help us prepare meals. A few hours of your time can make a lasting impact on someone’s journey toward stability and independence.
