Explore available housing options and connect with transitional, recovery, or community-based housing resources when appropriate.

Services and resources vary based on individual circumstances, eligibility, location, partner availability, and available funding.

UP TO 2 YEARS
OF FOLLOW-UP CONNECTION
OTWH's reentry model recognizes that progress takes time. Participants may remain connected through follow-up for up to two years after the more intensive stages of case management, helping maintain a pathway back to support and resources as new needs arise.
The type, frequency, and duration of support varies based on individual needs, program participation, capacity, and available resources.
